Proliferating Trichilemmal Tumor



Abstract
Proliferating tricholemmal tumor (Pilar tumor of the scalp) is a benign neoplasm of outer hair root sheath cells, characterized by abrupt keratinization in tumor cell nests and sometimes confusing with genuine malignancy.
Herein reported a case of trichilemmal tumor of child fist sized mass (5 x 6 x 4cm) in the left posterior inferior auricular area with mild tenderness in a 26 year old male. The mass was slowly and progressively enlarged with recent rapid growth., The tumor is movable and located in the subcutaneus and completely excised. Cut surface exhibits lobulated and granular with yellowish white appearance. Microscopically the tumor is composed of interlacing strands and nests of basaloid cells with central abrupt keratinization. Focal calcification and squamous eddies as well as a few atypical cells were also seen. But there is no evidence of infiltrative growth to surrounding tissue. Post-operative course was uneventful.
